NON OTTAWA-CARLETON DISTRICT SCHOOL BOARD STUDENT REGISTRATION INSTRUCTIONS

To register your child at A.Y. Jackson Secondary School, please call 613-836-2527 to make an appointment.  If you are new to the area and do not know your child’s designated school, please access the online School Locator.

For families wishing to register their child at A.Y. Jackson Secondary School from another school board or province the following is required:

 

  • Contact school at 613-836-2527 to make an appointment.

  • Ensure to bring proof of age in the form of a birth certificate, baptismal certificate, registration of birth card, or passport. Registration cannot be completed without one of these forms of identification.

  • Ensure to bring proof of address in the form of a driver’s license, utility bill, car lease, house lease, tax bill, credit card bill, bank statement, etc.
  • Ensure to bring your child’s last report card, credit counselling summary or transcript and an IEP or IPRC if applicable.

  • Ensure to bring your child’s Ontario health card and immunization record.

Ottawa Public Health requires proof that your child has received proper immunization. All children attending school in Ontario must be immunized against diphtheria, tetanus, polio, measles, mumps, and rubella, unless there is a valid exemption. If your child has lived or travelled outside Canada or in a First Nations, Inuit, or Métis community in Canada for three months or longer during the past five years, you will need a Certificate of Immunization and Tuberculosis Assessment for School Entry issued by Ottawa Public Health before starting school. If you are new to Canada, have a school-aged child, and English is not your first language, the Ottawa-Carleton District School Board’s Family Reception Centre can assist you with enrolling your child in school. The Centre’s staff will provide student assessment and guidance for English language learners and their families.
